Electronic Medication Cards

Electronic medication cards – living care documents, from first prescription to dispensed dose

Designed for clinicians, pharmacists and healthcare teams in Iceland and comparable Nordic markets alongside a national prescription registry and drug catalogue.

The Challenge

Disconnected & static medication records leave teams without a clear current view

Clinicians and pharmacists can be working from different versions of a patient’s medication status, relying on phone calls, faxes and manual reconciliation to fill the gaps. Prescription registries may go unchecked until dispensing, while traditional medication cards often provide only a static snapshot of one episode rather than the patient’s complete medication history.

Our SOLUTION

eMed creates one shared medication record, kept current across every care setting & transition

Clinicians can create, modify and stop prescriptions directly against the patient card, while pharmacists reviewing them in Atlas can see the full history of changes and previous episodes before confirming dispensing. This gives each participant the clinical context they need at the point of care, whether supporting prescribing, medication review or multidose dispensing.

Core Capabilities

Functionality that keeps medication information moving

Prescription Registration

Clinicians create and adjust prescriptions directly against the patient record, with every change tracked by user, timestamp and clinical context. Only prescriptions activated by the prescriber progress to dispensing.

Mandatory Pharmacist Review

Before a prescription progresses to dispensing, pharmacists complete a structured clinical review, with interaction warnings, allergy alerts, dose validation, expiry checks and national registry rules surfaced in one place. Dispensing cannot proceed until a required review is complete.

National Registry Integration

Prescriptions can be fetched, imported, reserved and validated against a national prescription registry, with rationing rules applied automatically and registry information available directly within the medication card.

Patient Medication Journal

Where configured, the patient’s medication history follows them across episodes and care settings – from nursing homes and primary care to hospital care. Prescriptions, changes and fixed-medication regimes remain available in one continuous record.

Multidose Plan Generation

Confirmed prescriptions are translated into a structured multidose plan, with products selected against the national drug catalogue, substitutions applied and pouch-level plans generated for the dispensing period before being passed to the configured dispensing platform.

EPR & Standalone Deployment

Runs embedded in an EPR or as a standalone module for nursing homes and primary-care institutions – same prescription model, same audit trail, same clinical-review logic in both contexts.

Solution Impact

Practical improvements demonstrated in live care settings

eMed can operate independently or integrate with existing EMR systems. In Iceland, it is commonly integrated with Saga, with benefits reported through live use including at Hrafnista, eMed’s largest client, supporting around 870 care-home beds.

Enhanced patient safety

With automatic interaction and duplicate-dose checks help identify medication risks as prescriptions are entered.

During real implementations, serious interactions have been identified that required immediate clinical action, while electronic information exchange also reduces the risk of transcription errors, helping to support safer pharmacotherapy in day-to-day care.

Streamlined medication ordering

Electronic orders can be sent directly to dispensing companies and pharmacies from within eMed, keeping medication changes and ordering in the same workflow.

Medication authorisation requests that previously required separate manual communication can be handled directly within the system, simplifying administration and saving clinical time when prescriptions change.

Improved interoperability

Integration with EMR systems brings relevant patient information, including admission details and allergy records, into eMed where it can be used alongside the medication record.

This gives nurses, doctors and pharmacists easier access to the information they need without repeatedly moving between separate systems, supporting a more joined-up medication workflow.

Clearer medication oversight

Integration with medication administration records and electronic ordering helps maintain a clearer view of medication information, updates and medication packages throughout the process.

This supports day-to-day oversight for healthcare professionals while helping organisations maintain consistent medication records and meet regulatory requirements.

For your team

Supporting different roles across your organisation

With the implementation of eMed, a comprehensive overview of patients’ medication use is achieved. Integration with Saga [EMR system] has simplified daily workflows and reduced the risks associated with manual data entry between systems.

During implementations of eMed, cases of serious drug interactions were identified automatically by the system that were not detected in previous workflows. Electronic ordering to pharmacies has further improved safety and simplified medication inventory management. We see improved traceability and safer information flow, which benefits healthcare institutions and nursing homes both in daily service delivery and in monitoring medication management.

Why eMED

Built for continuity, compliance & clinical oversight

Medication cards that stay current

eMed’s medication card is actively shared by clinicians and pharmacists together, across every care setting and episode. The patient’s full medication picture is always current – not assembled from fragments at the point of need.

Native regulatory fit

National drug catalogue, prescription registry logic, substitution rules and rationing are designed into the platform. The difference shows in clinical accuracy and regulatory compliance, not just configuration menus.

Pharmacist oversight guaranteed by design

Pharmacist review is a structural gate. Every prescription passes through mandatory review before it reaches dispensing. The prescriber’s authority is preserved; the pharmacist’s role is to ensure safe dispensing of what’s been prescribed
